Resources for Food Service Operators
The Michigan Modified 2009 FDA Food Code, adopted by the State of Michigan as of October 1, 2012, requires all employers and employees to complete employee health reporting forms. There are some diseases that are readily spread through food requiring employees be restricted from access with food or even excluded from the work place if they are infected. These illnesses include Shigella, E. coli, Salmonella, Hepatitis A and Norovirus. Employers must use the health reporting forms provided below.
